This resource looks at the importance of filling in a form correctly with personal details.

This learning resource has four parts that can be completed as individual sessions or grouped together depending on the target group or time allocated for the lesson.

Resources required:

Activity One – Scenario and Group Discussion – butcher’s paper or a Smart board.

Activity Two –Learning game – Print off flash cards and laminate for repeat use (appendix 1) and butchers paper or a Smart board to keep score.

Activity Three – Exercise filling in forms – Date and personal information, you will need to print off appendix 2 and 3.

Activity Four – Understanding of knowledge check, 2 forms for completion as individual exercise, you will need to print off appendix 4.

Note: keep the complete form and add to the student’s portfolio as evidence of completing a form correctly.
